Clindamycin phosphate is a water soluble ester of an antibiotic which is produced by a 7(S)-chloro-substitution of the 7(R)-hydroxyl group of the parent antibiotic lincomycin. Comprising of carbomer 940, edetate disodium, methylparaben, poloxamer 124, propylene glycol, purified water, and sodium hydroxide along with Adapalene, it is used for the treatment of acne vulgaris. It comes as a gel under the brand name of Deriva C for topical use.

The Deriva C gel (Clindamycin/ Adapalene gel) is indicated for the treatment of acne vulgaris.

How It Works

Cellular differentiation, keratinization, and inflammatory processes represent important features in the pathology of acne vulgaris and Adapalene acts as a modulator of these processes and acts on retinoid receptors by binding to specific retinoic acid nuclear receptors and thus helps in normalizing the differentiation of follicular epithelial cells resulting in decreased microcomedone formation and curing the acne vulgaris.

Dosage and Administration

The topical gel should be used as directed by your physician or you may follow the instructions inscribed on the instruction manual which comes along with the Deriva C gel.

The affected area should be cleaned and dried up before applying the gel. A thin coat of the Deriva C gel should be applied on the affected area and it should be applied two times in a day. After applying the gel on the affected area, you must get your hands washed properly with soap. The gel should not be swallowed and should be kept away from coming in contact with eyes as it is indicated for external use only. If accidentally, it gets it your eyes or mouth, then seek immediate medical help.

Side Effects

Because Deriva C gel is a topical for skin, its most common side effects are associated with skin like burning, itching, peeling skin, reddened skin and skin dryness.

The symptoms of diarrhea, bloody diarrhea and colitis (including pseudomembranous colitis), abdominal pain and gastrointestinal disturbances as well as gram-negative and folliculitis may be other side effects of the topical gel. Some side effects disappear even requiring no treatment but the persistence of any side effect for long should be immediately reported to your doctor and adequate recovery measures should be taken.

Drug Interaction

Patients who are being administered with drugs or any kind of topical having neuromuscular blocking properties should use, Clindamycin/ Adapalene gel, with caution as it may enhance the action of other neuromuscular blocking agents.

Because the Clindamycin/ Adapalene gel contains an alcohol base which will cause burning and irritation of the eye, if it comes in contact with eyes, the gel should be avoided from coming in contact with eyes. In the event of accidental contact with sensitive surfaces (eye, abraded skin, mucous membranes), bathe with enough amounts of cool tap water. While applying around the mouth, one has to be careful enough to not to let it get into the mouth. It may cause harm as it has been only designed for external use.

Pregnant women can take this drug only after obtaining the permission of a doctor.

Nursing Mothers

Nursing mothers can take this drug only after consulting their physicians as studies have revealed that adapalene is excreted in breast milk.

Pediatric Use

Children under the age of 12 years should not be prescribed with this medication as its safety and effectiveness in relation to them have not yet been established.

For individuals possessing a history of hypersensitivity to clindamycin or lincomycin or any other component of the Deriva C gel should be contradicted from using the gel. Also patients who have a history of regional enteritis or ulcerative colitis, or a history of antibiotic-associated colitis are also contradicted from using the topical.

The gel may be stored at controlled room temperature of 68 degree to 77 degree of Fahrenheit or 20 degree to 25 degree Centigrade. Keep the away from children and direct heat and light but do not freeze.
