Introduction

Adalat (nifedipine) belongs to a class of drugs called calcium channel blockers. It is a yellow crystalline material, nearly insoluble in water but soluble in ethanol. Besides active ingredient of nifedipine, it also has inactive ingredients such as lactose, silicon dioxide, corn starch, hydroxypropylcellulose, microcrystalline cellulose, crospovidone, and magnesium stearate.

Indication

Adalat (nifedipine) is medication for treatment of high blood pressure (hypertension). By lowering blood pressure, it can decrease the risk of developing health problems, such as a stroke, heart disease, or congestive heart failure, which can result from lasting high blood pressure.

Dosage

The dose of Adalat (nifedipine) will differ basing on many factors, including medical conditions and other medications you may be using.

Swallow Adalat capsules whole with a little liquid, either with or without food. The total daily dose should not be more than 60 mg. A starting dose of 5 mg is recommended at every eight hours with subsequent titration of dose depending on response. The dose can be increased to a maximum of 20 mg every eight hours.

In the elderly, a lower maintenance doses may be required compared to younger patients.

Patients with liver dysfunction must be carefully observed, and in severe cases, a lower dose can be used. Avoid taking it with grapefruit juice.

Side Effects

Usually adverse effects are minor and either require no treatment or can easily be treated by you or your healthcare provider. However, it is not that everybody who using the medicine will experience side effects. Some adverse effects that are possible in patients with high blood pressure are fatigue, extremely low blood pressure, body weakness (asthenia), nausea, diarrhea, vomiting, dizziness, headache, cough; rash etc. patients who take this drug for congestive heart failure may experience chest pain, kidney problems, abdominal pain (stomach pain), diarrhea, extremely low blood pressure, headache, dizziness etc.

Drug Interactions

This drug potentially interacts with numerous other medicines. Some of these drug are diuretics, certain antibiotics or antifungals, nonsteroidal anti-inflammatory drugs (nsaids), potassium supplements, grapefruit juice, potassium-sparing diuretics, quinidine, lithium (eskalith®, lithonate®, lithane®, lithotabs®).

Special Precautions

Adalat may be used together with beta-blocking drugs and other antihypertensive agents but it may likely cause an additive effect resulting in postural hypotension. It will not check possible rebound effects after termination of other antihypertensive treatment.

Caution is needed in patients with very low blood pressure.

Treatment with short-acting nifedipine may cause a great drop in blood pressure and reflex tachycardia, which can lead to cardiovascular complications, for example myocardial as well as cerebrovascular ischaemia.

It may cause angina pectoris very rarely, especially at the start of the treatment.

The patient need careful monitoring of blood pressure when taking nifedipine with I.V. magnesium sulphate, due to the risk of an excessive fall in blood pressure, which could harm both mother and foetus. In patients with impaired liver function, a lower dose should be administered.

Contraindications

Adalat is contraindicated in malignant hypertension, in patients with known hypersensitivity to nifedipine, or to other dihydropyridines.

This drug should not be administered in pregnancy before week 20 and during breastfeeding.

Adalat is forbidden in cases of cardiogenic shock, unstable angina, clinically significant aortic stenosis, or during or within 4 weeks of an acute myocardial infarction.

It can't treat acute attacks of angina. This medication is contraindicated for secondary prevention of myocardial infarction and concomitantly with rifampicin.

Storage

Keep the capsule away from light and moisture and at controlled room temperature 20° to 25°C (68° to 77°F).
