Introduction

As a meglitinide antidiabetic, Repaglinide is prescribed for patients suffering from type 2 diabetes who cannot control the blood glucose by diet or exercise. It stimulates the release of insulin from the pancreatic gland and reduces blood glucose. It can be used concomitantly with other antidiabetic medicines or alone. Repaglinide is a white to off-white powder and is available in the form of tablets for oral administration under the trade name of Generic Prandin in the strength of 2mg. The Generic Prandin tablet contains calcium hydrogen phosphate (anhydrous), microcrystalline cellulose, maize starch, polacrilin potassium, povidone, glycerol (85%), magnesium stearate, meglumine, and poloxamer along with iron oxide which is a coloring agent.

Drug Indications

Belonging to the class of megnitinides, Generic Prandin tablet is indicated as an adjunct to diet and exercise to improve glycemic control in adults suffering from type 2 diabetes mellitus.

Mechanism of Action

The beta cells in the pancreatic islets function to stimulate the release of the insulin from the pancreas due to the action of Generic Prandin which results in the fall in glucose level in the blood. By binding at characterizable sites, Repaglinide closes the dependent potassium channels in the β-cell membrane and the beta cells get depolarized due to the blockage leading to opening of calcium channels. Insulin secretion is induced by the resulting increased calcium influx.

Dosage & Administration

The dosage of Generic Prandin tablets are dependent and can be best advised by your physician. Strict instructions of your doctor should be followed for taking these tablets.

Following is the general dosing of these tablets. Adult patients who were never given treatment with these tablets or those who have HbA1c lower than 8% may be administered with a dose of 0.5mg at each meal. But the patients who have already had treatment with blood glucose-lowering drugs or individuals who have HbA1c greater equal to 8% should be given a dose of 1 or 2 mg after each meal.

Dose Adjustment

Dosing adjustments should be determined by fasting blood glucose response. An adjustment in doses may be required by patients who have satisfactory pre-meal blood glucose levels but overall glycemic control (HbA1c) is inadequate. Such patients may be administered with a dose of 4 mg per day. Preprandial dose of a maximum of 16mg per day may be given in divided doses of 2, 3 or 4.

Side Effects

The most severe side effects of Generic Prandin tablets which lead to the discontinuation of these tablets are hyperglycemia, hypoglycemia, and other related symptoms to these. Glyburide and glipizide patients have experienced moderate side effects with this drug. Common seen side effects of this drug are alopecia, hemolytic anemia, pancreatitis, Stevens-Johnso Syndrome, and severe hepatic dysfunction including jaundice and hepatitis. Immediate reporting regarding the side effects should be done to your doctor.

Drug Interactions

Repaglinide is interactive with some antifungal agents like ketoconazole, itraconazole, and antibacterial agents like erythromycin leading to an increase in the plasma level in the body. Trimethoprim increases its level in the blood. Gemfibrozil also enhances the exposure of these tablets, so it should be carefully administered with this drug.

Drug Contraindications

Repaglinide pills are contradicted from being administered to patients suffering from diabetic ketoacidosis, with or without coma, which should be treated with insulin. Type 1 diabetes should not be prescribed with the drug. Administration of gemfibrozil along with these tablets is contradicted. Person having hypersensitivity to these tablets or any other component of these tablets should not consume these tablets.
