Proctosedyl Ointment is offered in the form of odorless and yellowish-white translucent greasy ointment. The active ingredients in this drug include cinchocaine hydrochloride of 5 mg, hydrocortisone of 5 mg, Framycetin Sulphate BP of 10 mg and Aesculin of 10 mg. it belongs to the pharmacological classification of suppositories and anal ointments. Tubes hold 30 g ointment.

Indications

Proctosedyl Ointment is indicated to treat internal and external haemorrhoids and haemorrhoids post-partum. This drug is also used to treat peri-anal eczema, anal pruritus, anal fissures and proctitis and post-haemorrhoidectomy application to reduce pain and discomfort.

Contra-Indications


This ointment is contraindicated in patients with known hypersensitivity to any of the ingredients. Corticosteroids have caused teratogenicity in animals by dermal application. Therefore this medication should be avoided during pregnancy. This ointment should not be applied to treat herpes simplex, varicella, vaccinia or tuberculous infection of the anal region (chronic fissure).

Dosage and Administration


A suppository is inserted morning and evening, and after each stool. Apply the ointment in small quantity with the finger, on the painful or pruritic area, morning and evening and after each stool. For deep application attach cannula to tube, insert to full extent and squeeze tube gently from lower end whilst withdrawing. The ointment may be used separately or concurrently with the suppositories.

Side-Effects and Special Precautions


Long-term continuous use of Proctosedyl topical may give rise to atrophic changes in the skin causing thinning, dilatation of superficial blood vessels, and loss of elasticity, telangiectasiae and ecchymoses. There is more chance of such incidence if occlusive dressings are used. It may cause systemic absorption of corticosteroids. Other conditions like using it in large quantities or using the application on wide areas of the body, or applying to damaged skin; or applying strong topical corticosteroids may also lead to systematic absorption of the ointment.

Use of Proctosedyl Ointment may also cause depression of the hypothalamic-pituitary-adrenal axis apart from subsequent suppression of the adrenal gland. Children are at greater risk to suffer from this disease. Use of this topical may hold back growth and bring a Cushingoid state. This drug may rarely cause benign intracranial hypertension.


Local anaesthetics are potent sensitisers and may give rise to allergic reactions. Hence the use of these topical to the skin for extended periods or to wide areas are needed to be avoided.


Patient may experience skin sensitization if use local use of aminoglycosides for a long period. The topical application of aminoglycosides to mucous membranes has caused Ototoxicity. It may also bring about reversible nephrotoxicity.

Over dosage of aesculin have been reported to cause muscle twitching, dilated pupils, lack of co-ordination, vomiting, paralysis, diarrhoea, and stupor.

If topical application of this ointment causes a secondary microbial skin infection, allow appropriate concomitant anti-microbial treatment.

In case of the patient experience any symptoms of over dosage; he/she must consult with the doctor.
